BEOHARI /SHAHDOL: Back pain (Backache) is the most common disorder that is more common in women than men. It is not a disease it is due to condition in body. Pain is present in posterior region of trunk from neck to pelvis, in site of lumber region, sacrum, sacroiliac joints & coccyx. The intensity of pain varies from a little discomfort to complete disability to move or sit up in bed.
The causes of backpain may be devided into the following categories-
GYNAECOLOGICAL CAUSES– In the majority of women backpain is usually present with gynecological ailments. In case of women, the following gynecological causes should also be considered-
- Chronic inflammation of uterus or cervix- Chronic inflammation in uterus or cervix also causes pain in back as a reflex symptom.
- Postoperative backache- This is due to weakness & strain in sacroiliac region.
- Premenstrual sickness- It is due to pelvic congestion, there is backache, which in some cases is very severe.
- The site of backache is gynecological cases is mostly sacrum or coccyx. Gynecological conditions can be accompanied by backache.
- Prolapsus & retroversion of uterus, both of these conditions create abnormality at the pelvic ligaments & this causes backache.
- Tumors (Benign or Malignant) in the pelvic region are usually accompanied by backache.
- Fibroid growth- Fibroid or any other growth in the uterus, fallopian tube or an ovary may cause backache.
- Profuse leucorrhoea mostly accompanied with backache.
- NON ORTHOPAEDIC CAUSES-
- Pain in the renal region- It is mostly unilateral & arises at the loins. Other symptoms of renal insufficiency may be present.
- Rectal pain- Pain in rectum due to constipation, rectal spasm, fissure or chronic hemorrhoids & carcinoma cause discomfort & pain in the sacral region.
- ORTHOPAEDIC CAUSES- Orthopedic causes play an important role in the production of backache.
- a) Spinal caries- This disease of the spinal bone maybe due to tuberculosis or old injury. It is often accompanied by severe pain in back.
- b) Compression of the spinal cord- It is due to ossification in the bone or some growth impinging on the nerve tissue. It creates chronic & long lasting backache.
- c) Pain in sacroiliac joint- It is often present during pregnancy & maybe present even after the delivery.
- d) Spinal arthritis- Any kind of inflammation of spinal joints such as rheumatic, gouty, osteo- arthritic etc causes pain at the site of the trouble. It may be accompanied by local tenderness.
- e) Coccygodynia- Coccygodynia generally follows labour especially a hard one. Tenderness is elicited on movement of coccyx or pressure on end point.
- f) Many people while sitting, standing or walking assume defective postures. This is the case especially with obese. Continuous habitual adoption of wrong posture becomes a predisposing cause of backache due to wrong posture there is muscular & ligamentary strain on the back that strain causes pain.
